Use this form when a family needs the next ABA call

A referral form should not bury a busy office in extra steps. Budding Futures needs the family contact, city, payer, and why your team is sending them. If the parent is already on a waitlist, write that. If the diagnosis report is not finished yet, write that too.

Who usually uses this page?

Pediatric offices use it after a diagnosis visit. Schools use it when parents ask about ABA outside the school day. SLPs, OTs, and care teams use it when home routines, safety, or communication are blocking progress.

What we do after the form arrives

Budding Futures calls the parent or guardian and asks about home life, insurance or Medicaid, records, and timing. If our team may fit, we explain the next step in plain language.

What makes a form useful

A good form does not ask your staff to solve the whole case. It gives the ABA team enough to call the family and find out what is real. A parent may say Medicaid, but not know whether it is Health First Colorado. A school team may know the child needs help at home, but not know whether ABA is the right fit yet. That is fine.

When to use the phone instead

If a parent is upset in your office, if a guardian is hard to reach, or if the family is trying to leave another waitlist, call after sending the form. A short heads-up can help our team understand why the family needs a call quickly.
